Stevens Johnson Syndrome and Toxic Epidermal Necrolysis

Dilantin

On November 24th, 2008 the FDA issued a MedWatch Alert informing healthcare providers of a potential increased risk of serious skins reactions associated with the use of Dilantin®, Phenytek®, Cerebyx® and Tegretol® by certain patients of Asian descent. In December, 2007, the FDA described an increased risk of SJS and TEN associated with another anti-epileptic drug, Tegretol® (carbamazepine) in patients of Asian descent.

Dilantin®, Phenytek® and Cerebyx® Dilantin® and Phenytek® are widely prescribed anti-epileptic medications commonly referred to as phenytoins. Cerebyx®, also an anti-epileptic treatment, is an injectable medication and commonly referred to as fosphenytoin.

Each of these medications is used as an alternative to Tegretol® (carbamazepine), an anti-epileptic/anti-convulsive treatment that has been associated with SJS and TEN. However, there have been reports that patients taking Dilantin®, Phenytek® and Cerebyx® have also developed SJS and TEN. Patients of Asian descent may be at increased risk of SJS and TEN when taking these medications.

Stevens-Johnson Syndrome (SJS)/Toxic Epidermal Necrolysis (TEN) SJS and TEN are related, but distinct serious skin disorders the initial onset of which produce cough, body aches, fever and headache. A red skin rash then appears on the face and/or torso which oftentimes spreads and forms blisters.

With SJS, blistering of mucous membranes such as the lips, mouth, eyes and other such "mucosa" appears with perhaps 10% of the body being affected. Blistering also occurs with TEN. However, large areas of skin become very painful and will peel off – sometimes with even the slightest touch.

SJS and TEN can be life-threatening so early detection and treatment are critical. If you are currently taking Dilantin®, Phenytek® or Cerebyx® and have questions or concerns about your medications, you should contact your healthcare provider/prescriber.
